Optimal cardinality of a standard adaptive finite element method

We consider a general linear elliptic second order PDE and show optimal cardinality of a standard AFEM consisting of the loop SOLVE -> ESTIMATE -> MARK -> REFINE. It makes use of the standard procedures

SOLVE: calculates the Galerkin approximation.
ESTIMATE: computes the standard residual based estimator.
MARK: marks elements according to Doerfler's strategy only for the estimator.
REFINE: refines each marked triangle by newest vertex bisection at least once.
